Kit includes: One at-home blood sample collection device (Tasso+) with microtainer and label, one requisition form to be completed and returned with your sample, one biohazard bag with label, one instructional pamphlet, one alcohol wipe, one heel warmer, and one pre-paid shipping label for returning your samples (in the same box you received the kit).
